Document Checklist – Self-Employed

Please have the following documents ready for upload:

1. NRIC copy (front and back);

AND

2. One of the following documents:

i. Latest 2 years LHDN e-filing acknowledgement; OR

ii. Latest 2 years EPF statements

AND

3. One of the following documents:

i. Professional Certificate / Operating License showing company has been established for more than 2 years and company shareholding / ownership; OR

ii. Companies Act 1956: Form 9, 24 & 49; OR

iii. Companies Act 2016: Section 17, 58 & 78; OR

iv. Any other supporting documents indicating company ownership and establishment date
